This year, our focus remains ensuring everyone at Jack Hille Middle School can learn and work in a healthy and safe environment. In addition, our goal is to provide continuity of education with as little disruption to the learning process as possible.
Therefore, in order to more effectively monitor the daily health and safety of our students, we are asking families to submit their child’s proof of vaccination card. This will allow our administrators to provide more effective contact tracing, potentially allow more students to be within three feet of each other and is a requirement for students participating in any athletics.
You can easily submit your child’s proof of vaccination card at https://bit.ly/d142-vaccinationcard. Please use your child’s iPad to fill out this form and upload the proof of vaccination card as this form requires a District 142 account.
The deadline to submit this information is Friday, September 3 at 4:00pm.
